Acetabular Revision using An Acetabular Reinforcement Device: -4 to 7 year results-

Abstract
Purpose: To evaluate the intermediate term clinical and radiographic results of acetabular revision surgery, with massive bone loss, using ARD (Acetabular Reinforcement Device).

Materials and methods: The study included 14 cases of acetabular reconstruction, using an ARD, with a bone graft, performed between May 1992 to February 1998. The Patients were followed for between 4 to 7 years. Clinical evaluations were performed according to the Harris hip score (HHS), and the radiographic loosening was measured according to Gill¡¯s criteria.

Results: The HHS improved from 37.2, preoperatively, to 87.4 by the last follow up. Postoperatively, the hip center was nearly restored to a normal anatomic hip center in all patients. There was 1 probable loosening and 2 possible multiple loosenings. No loosening occurred in cases where more than three screws were fixed. The grafted bone was shown to be incorporated in 13 cases.

Conclusion: The clinical and radiographic results of an acetabular revision, with massive bone loss, using an ARD, appeared to be unhelpful in terms of implant failure at the 4 to 7 year follow-up. However, an ARD, with bone graft, can achieve the successful augmentation of the bone stock of the acetabulum, and may enable the restoration of the hip center to the anatomic position. An ARD, with an inferior hook, fixed with 3 or more screws is recommended for the avoidance of radiographic loosening.
